Cranial Spinal Irradiation is one of the most challenging cases in radiation therapy treatment planning.  In this presentation, two different RayStation clinics will share their experience in streamlining their workflow through scripting, use of protocols, standardization and more.  These happen to be two proton facilities, but the knowledge shared is applicable to any medical dosimetrist. 

The presentation will cover the rationale for expedited CSI treatments while also diving into robust optimization as a means of covering the field junctions.  Both use scripting and will emphasize you don’t need to be a scripting expert in order to find these efficiencies.   

Learner Outcomes:

  1. Explain the rationale for Emergency-Expedited Proton CSI
  2. Outline the evolution of CSI planning, culminating the discovery of the novel RayStation Robust Optimization application. 
  3. Detail the utilization of RayStation’s Robus optimization tools, and others, to achieve expedited robust proton CSI treatment plans.
  4. Evaluate plan quality and introduce future work, including automation enhancements and alternative planning methods.

Alexander Goughenour, BS, CMD, Rev, is a father, husband, and medical dosimetrist.  Alexander specializes in proton therapy treatment planning with clinical experience from over ten North American proton centers, he is most passionate about systems engineering, workflow optimizations, and general efficiencies.  

Alexander’s favorite color is light tan and his favorite animals are puppies. He likes serving the Lord, hiking, and playing volleyball.

Sara St. James, PhD, DABR, is an Associate Professor of Radiation Oncology at the University of Utah. She completed this medical physics residency at Harvard Medical School
